Instead, it produced a fragmented ecosystem of competing EHR platforms—Epic, Cerner, Allscripts, and dozens of others—that were not designed to communicate seamlessly with one another.
Today, a patient who received care at a community hospital in Texas, relocated to Illinois, and now sees specialists in New York may have active medical records on three incompatible systems, none of which automatically shares information with the others. Each institution holds a partial truth. No single provider holds the whole picture.
The Real Cost of Disconnection
The consequences of this fragmentation extend well beyond inconvenience.
Duplicate testing is among the most immediate and measurable effects. When a specialist cannot access prior imaging or laboratory results, they frequently order new ones—not out of negligence, but because practicing medicine without complete information is inherently risky. A 2019 report from the Pew Charitable Trusts estimated that incomplete or inaccurate patient matching across health systems contributes to billions of dollars in unnecessary testing annually.

Why Interoperability Remains Unfinished Business
Federal policy has moved toward addressing these gaps. The 21st Century Cures Act, signed into law in 2016, established new requirements for health information exchange and explicitly prohibited practices—collectively termed "information blocking"—through which healthcare organizations restrict access to patient data for competitive or financial reasons.
The Office of the National Coordinator for Health Information Technology (ONC) has continued to issue rules designed to strengthen interoperability standards, including requirements for certified EHR systems to support standardized application programming interfaces (APIs) that allow patients and authorized providers to access health data more freely.
Progress has been real. But implementation has been uneven. Smaller practices and rural health systems often lack the technical infrastructure to participate fully in health information exchanges. Legacy systems remain in use at institutions that have not yet completed EHR migrations. And the cultural and competitive reluctance to share patient data across institutional boundaries has not been fully resolved by regulatory mandate alone.
The result is a system that is better than it was a decade ago, but still far from the seamless, patient-centered information environment that safe, coordinated care requires.
What Patients Can Do Right Now
While systemic change continues to develop, patients are not without agency. Several practical steps can meaningfully reduce the risks associated with fragmented records.
Request and retain your own records. Under the Health Insurance Portability and Accountability Act (HIPAA), patients have a legal right to obtain copies of their medical records from any provider. This right extends to electronic copies when the records are maintained in an EHR. Submitting written requests to each institution where you have received care—and maintaining a personal health file—creates a consolidated resource you can bring to any new provider.
Use patient portal access proactively. Most hospital systems and large practices now offer patient portals through which individuals can access visit summaries, laboratory results, medication lists, and imaging reports. Downloading and organizing this information regularly prevents gaps from accumulating over time.
Leverage Apple Health Records or similar aggregation tools. Apple's Health app, and comparable tools on other platforms, can connect directly to participating health systems and aggregate records into a single, portable format. While not universally supported, coverage is expanding. Patients who use these tools gain a degree of record portability that the institutional systems alone do not provide.
Designate a primary care physician as your care coordinator. In the absence of a unified system, a trusted primary care physician who knows your full history can serve as a human integrator of your care. Ensuring that all specialists send visit notes and test results to your primary care provider creates at least one point of record consolidation within the clinical system.
Be explicit with every new provider. When establishing care with a new physician or specialist, explicitly describe where you have received prior care and request that they initiate record requests before your appointment.
